  • Please visit http://www.usa.philips.com/c-m-pe/teeth-whitening rebate to subscribe for a rebate.
  • Subscribe to rebate and you will receive an email with a rebate form and a promotional code. Save the email from Philips Zoom that includes your promotional code.
  • Print out rebate form and mail back completed rebate form including the original sales receipt and Proof of Purchase (cut-out the lot number sticker from the Philips Zoom chairside light-activate whitening gel 25% HP – ask your Dental Assistant for this!)
  • Mail your cut-out lot number, dated receipt and the form below to:

  • Lost or stolen, damaged or expired rebate checks/items will not be replaced.
